Collect your evidence 

*All applications must be submitted prior to attending your IRATA training course.

Graviteq will not be able to process your CSQ Funding application if you do not provide all of the required evidence.

 

Required evidence for all applicants (one from each category):

1. Consent 

• A completed CSQ consent form (link above)

2. Proof of Australian rights to work (in construction)

• A copy of your birth certificate or passport

• A Medicare card

• A copy of your visa (if you are not an Australian citizen or permanent resident)

3. Eligible worker

• An employer letter (template above)

• Relevant current licence

• Statutory Declaration (template above)

• QLeave Building and Construction Member Statement

Or for employers:

• ABN/ACN registration showing ownership 

• and Invoices for recent work and construction site address

4. Government issued photo ID

• Driver license

• Passport

5. Course confirmation

• A paid in full IRATA (email ticket confirmation) view example

6. Graviteq processing fee

• A paid in full CSQ processing fee (email ticket confirmation) view example

Successful course completion

Note: This will be attained from your independent IRATA assessor

• IRATA assessment form (yellow of white copy). view example
